To Operate A Coordinated Program Of Home Care With Provision For Inpatient Care For Terminally Ill Patients And Their Families. Care Must Be Provided By A Medically Directed Interdisciplinary Team Consisting Of A d Physician, A Nurse, A Social Worker, A Clergy Member, And A Trained Hospice Volunteer. The Hospice Program Of Care Provides Services To Meet The Physical, Psychological, Social, Spiritual And Special Needs Of Patients And Their Families Experienced During The Final Stages Of Terminal Illness And During Dying And Bereavement. Services Must Be Available 24 Hours A Day, 7 Days A Week.
